For each consequence tier, assign a named accountability owner: the human whose judgment governs the final decision. Accountability cannot be distributed across committees or left to "the system."
Design the specific workflow mechanisms that give accountability owners the ability to approve, modify, or reject AI recommendations — with documented reasoning captured at each decision point.
Define when and how decisions escalate to higher accountability levels: thresholds for escalation, escalation destination by decision type, and documentation requirements for escalated decisions.
Build the logging infrastructure that records who reviewed, who approved, what was overridden, and what the reasoning was — creating a complete accountability chain for every consequential AI-influenced decision.
